they where doing an upright on a t\t for a class when they started the lift this block failed and shot it up and over the casulality and landed between the tractor and trailer, when they found both pieces it was discovered that the piece on the sheve where the pin screws down onto it had two small cracks that caused the failure. If this is incorrect i hope that someone will correct me
